DIXON — The Community that Cares Project is such a wide-ranging and ambitious project, that sometimes even project staff members and community partners in the project are amazed at the progress that has been made over the past year and the milestones that have been passed in trying to better coordinate care for children and families.
The Community that Cares-System of Care Project is a grant funded project. The funding comes from the Illinois Children’s Healthcare Foundation. Community that Cares (CTC for short) has just finished up a productive first year of implementing an integrated network of care for children and their families.
This includes improved coordination of care across systems such as education, mental health, public health and other child and family-serving providers.
